    Preventive Health

    In today’s healthcare environment it’s important for you to have a relationship with a primary care provider, such as the physicians at Medical Group of the Carolinas - Family Medicine - Five Forks-Pelham. This relationship allows the physicians to better meet your needs and gives you a partner who knows how to navigate the increasingly complex world of healthcare. Our physicians will serve as your ally and advocate and work with you to detect and/or prevent issues that might affect your health and well-being.

    Call our office to schedule a preventive visit in which our physicians sit down with you for a conversation about your lifestyle and family history, then he or she will make recommendations, based on the information shared, to screen for diseases such as diabetes, heart disease and certain forms of cancer. If it has been a while since you had a checkup, we encourage you to schedule a routine health-maintenance exam, where in addition to a conversation about your lifestyle and family history, a complete physical exam and lab work will be done. With this information, your physician will make recommendations for lifestyle changes and for additional tests, if necessary.

    Scheduling Health Information Guidelines

    Some general guidelines, regarding how often routine health maintenance exams should be scheduled are as follows; these are only guidelines that you should discuss with your physician:

    Children – Frequent visits to assess developmental milestones and give childhood vaccinations

    Adolescents – Annual visits to assess development and discuss health issues and behaviors

    Young Adults – Routine visits to discuss health issues and annual tests

    Middle-aged Adults – Annual visits are recommended to monitor issues such as blood pressure, cholesterol and weight, recommendations on additional screenings such as mammograms and colonoscopies are also discussed at this time

    Senior Adults – Regular visits to assess health habits and discuss disease prevention (Medicare has recently added an Annual Wellness Visit to its benefits)

    New Patients & Patient Information

    New Patients

    We welcome new patients. When calling to schedule a new-patient appointment, we will ask you some basic information to get to know you and prepare for your visit.

    If you are a new patient, please arrive 30 minutes early and bring any medications you take, your medical records, insurance card/s and a photo I.D.

    New patient paperwork that needs to be completed prior to your scheduled appointment can either be mailed to your home or downloaded here for your convenience.

    Patient Information

    If you have had a change in your health status since your last visit (such as a hospitalization, surgery, emergency room visit or other illness), please inform us when making your appointment, so we can obtain the necessary information prior to seeing you.

    Remember to bring either your medication bottles or an accurate list of all your prescriptions and supplements. If lab work is needed, you will be informed when scheduling your appointment whether or not you need to fast.

    After Hours

    You may call 864-849-9170 anytime, 24/7, to schedule or change an appointment.

    Electronic Medical Records

    We use MyChart, an electronic medical record (EMR) system that allows information to be accessed rapidly and shared with nurses, technicians, and other healthcare professionals involved in your care, and prescriptions to be transmitted directly to your pharmacy.

    Insurance & Billing

    We request payment of all office visits at the time of service. Co-pays are required for all insured and uninsured patients.

    If you have questions concerning fees, our office or Physicians Billing Service will be glad to assist you at 864-560-4304 or 877-596-2455.

    We are participating providers for most medical insurance plans. Because there are a wide variety of insurance options, please contact your insurance company directly or one of our patient access representatives to ensure that our practice is a provider for your plan. Please remember to bring your insurance card with you.
