    About Us

    A visit to your doctor doesn’t have to be a dreaded experience. When we created Medical Group of the Carolinas - Family Medicine - Five Forks, we listened to patients and designed our practice to make coming to see us as stress-free and relaxing as possible.

    We adapted concepts from the worlds of business, entertainment, architecture and manufacturing in designing this new healthcare experience. We spent more than a year working with leading researchers in health care to design a practice that blends technology, architecture and efficiency, while continuing our core value of patient-centered care.

    Patients are welcomed to Medical Group of the Carolinas - Family Medicine - Five Forks by greeters in our spa-inspired concourse. They are escorted to an assessment room, a multi-purpose room that is clinical, yet comfortable and inviting.

    Everything from registration to interaction with our doctors is done in the privacy of this room. Large, flat-screen monitors are present for patient education and can be used by the physician to communicate information such as recent lab work or other test results. Before leaving this room, we will complete your visit by assisting you in making your next appointment at a time convenient for you.

    If a procedure or more comprehensive physical exam is needed, standard, traditional examination rooms are available for use. Additionally digital x-ray and on-site lab services are available if needed.

    Our staff is committed to quality, patient centered health care. Patients are treated as family. Our physicians are respectful of you, your time, your wishes and are excited to be your partner on your personal healthcare journey.

    Our practice is a member of Medical Group of the Carolinas (MGC), a large multi-specialty physician group serving Upstate South Carolina and parts of North Carolina. MGC doctors are dedicated to meeting your healthcare needs, from wellness and prevention to the diagnosis and treatment of illnesses.


    Please arrive 15-20 minutes before your appointment to fill out your medical history and registration forms. You may also be provided a health assessment or questionnaire to complete in advance of your visit. To cancel an appointment, please notify us 24 hours in advance so we can make the appointment available to another patient.

    What to bring with you:

    • Current medications
    • Medication reconciliation form, if needed
    • Photo ID, insurance, Medicare or Medicaid ID card
    • Co-payment, if required by your insurance provider
    • Prior medical records, including documentation of any allergies, chronic conditions, treatments or surgeries
    • Questions for your doctor

    Current patients can schedule appointments by phone, or use the MyChart patient portal or app to see available times.

    Once you become a patient you will find that YOU are the most critical part of your care team. As a patient, you can obtain care and clinical advice during normal business hours by contacting your physician’s office. After business hours, our answering service will contact the appropriate person to handle your needs, as we have an on-call physician available 24 hours a day, seven days a week.

    Prescription Refills

    If you are a patient currently using MyChart, you may be able to request a refill through the app or online portal. For prescriptions not available in MyChart, please make an appointment so a physician can evaluate if the medications need to be changed. It could take up to 24-48 business hours for a refill to be submitted.

    Patient Records

    Our practices use an electronic health record system that allows for a seamless patient experience through improved communications, timeliness, patient information storage and portability. This system, called MyChart, will allow you to view medications, allergies, health issues and recent visits.

    New patients in our practices are asked to complete a medical release form before your appointment. The signed form gives us the ability to request medical records from other physician practices that you may have visited. This information provides our physicians with a more complete medical history. To obtain or transfer detailed medical records to another practice, please contact the Medical Records Office at 864-560-6273.

    Insurance & Billing

    We are participating providers for most medical insurance plans. Because there are a variety of options, please contact your insurance company or one of our patient access representatives to ensure our practice is a provider for your plan. Please remember to bring your insurance card with you.

    We request payment for all office visits at the time of service. Co-pays are required for all insured patients. If you have questions about fees, our office or Physicians Billing Service will be glad to assist you at 864-560-4413 or 877-596-2455.

    Our practice is recognized by the National Committee for Quality Assurance (NCQA) as a Physician Practice Connections®-Patient Centered Medical Home (PPC-PCMH). Our physicians and staff are dedicated to providing medical care tailored to you and your family's healthcare needs. Your personal healthcare team will work with you to make informed decisions on the most effective and appropriate treatment of care and help you set goals to improve your health and quality of life.

    Calling Us

    All telephone calls of a medical nature are reviewed by a nurse and are handled as promptly as possible. Urgent calls are given priority. In order to provide the best assessment and care for your health, we generally do not diagnose or prescribe treatment by telephone.

    If you or a family member experience an emergency after hours, please call 911 for assistance. If you become ill after hours but it is not a medical emergency, please call your physician’s office. Your call will automatically be routed to our answering service, which is available to assist you. Calls should only be made for health problems that cannot wait until normal business hours. It is our policy not to refill medicines or call in new prescriptions after hours.
